Essential Hypertension (initial)
-Two office measurements on two separate occasions confirm hypertension.
–ASCVD score of XXXX.
-12 lead ECG, urine albumin to creatinine ratio, blood workup including: CBC, ESR, creatinine, eGFR, electrolytes, HbA1c, thyroid profile, blood cholesterol levels, and serum uric acid.
-Will obtain ankle-brachial pressure index, if concerned for PAD.
-Patient educated and instructions provided regarding weight management, salt restriction, smoking management, adequate management of obstructive sleep apnea and exercise.
-Counselled on smoking cessation
-Will start ACEi/ARB +/- CCB. Will consider thiazide diuretics + CCB in AA. Re-evelaute in 12 weeks, and titrate medications based on response.
-Target BP <140/90.
-Secondary workup for BP with labs above.
