-DDx includes: Cellulitis vs abscess. 
-Obtain CBC, BMP, ESR, and C-reactive protein (CRP). 
-Obtain and follow blood cultures.
-MRI with contrast vs CT scan. Discuss with podiatry/surgeon.
-NPO at midnight. Will need bone biopsy and culture of biopsy, unless positive blood cultures.
-IV Vancomycin + Zosyn post biopsy, but will start empirically if septic. Narrow based on culture results. Antibiotics for 6 weeks.
-Follow CRP level to monitor treatment and response.
-If prosthetic joint present, will need removal. Antibiotics for 3-6 months.
-Need for surgical debridement. Consult placed.
-Wound vac and wound care consult.
